User Responsibilities

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of New Jersey, without regard to its conflict of law principles.

By clicking "I Agree," checking any box indicating your acceptance, using any other acceptance method provided through the service, or otherwise affirmatively accepting these Terms of Service, you confirm that you have read, understood, and agree to be bound by these terms. If you do not agree to these terms, you are not allowed to create an account. You also authorize any party who accepts these terms and conditions on your behalf to act as your agent.

This User Agreement includes a mandatory arbitration clause. Unless you opt out of arbitration in accordance with these terms within the specified timeframe, you agree that any disputes between you and us, or between you and the medical groups or providers, related to these terms or the service, will be resolved through binding, individual arbitration.

You also waive your right to a jury trial and to participate in a class action lawsuit or class-wide arbitration.
